Note: The csc scholarship covers the period of study as determined upon admission, without extensions. Scholarship recipients are required to adhere to the csc Scholarship regulations, and changes in institution, major, or study duration are generally not permitted.

| Study Plan Proposal | Minimum 1000 words, written in Chinese or English, detailing your major or research area. Must be signed by a DUT supervisor. |

| Foreigner Physical Examination Form | Complete all items in English with required stamps/signatures. Valid for six months. |
