Required Documents for Application
1. Passport Front Page (Valid for 1 year at least)
2. Highest Degree Notarized (If you are applying for a master’s degree, you need a bachelor’s degree; if you are applying for PhD degree, you need both a master’s and a bachelor’s degree.)
Note: If your degree is not completed yet, you need a HOPE CERTIFICATE or Provisional Certificate from your university.
3. Transcripts Notarized (If you are applying for a master’s degree, you need a bachelor’s transcript; if you are applying for a PhD degree, you need both a master’s and a bachelor’s transcript.)
4. Research Proposal (PhD, 1500-2500 Words) or Study Plan (Master’s, 1000- 1500 words)
5. English Proficiency Certificate.
6. Two Recommendation Letters (by Associate Professors or Professors).
7. Foreigner Physical Form
8. Non-Criminal Certificate (Latest Police Character Certificate)
9. CV
10. Photo (White Background)

Frequently Asked Questions
Make photocopies of all your academic degrees and transcripts (not the originals).
Visit your local district court (Kachehri) or a registered notary public office.
Find an official notary public or a lawyer/advocate who holds a notary public stamp.
Ask them to attest (stamp and sign) each photocopy to certify that it is a true copy of the original document.

2. Affix a recent passport-sized photograph of yourself in the designated space on the form.
3. Visit a local hospital or a registered physician who can perform the required laboratory tests and complete the medical examination.
4. Ask the doctor to fill out the form, perform the necessary tests, and then sign the form at the designated section. The doctor must also place their official stamp half on the photograph and half on the form (overlapping the photo and the form).
5. Request the complete laboratory test reports from the doctor as well. Some universities require these reports in addition to the form.
6. Fill in the form either on a computer or write clearly in capital letters to ensure all information is legible.
7. Ensure the form has the latest possible date, as the validity of this medical form is only 6 months from the date of issue.
- Obtain a Police Clearance Certificate (Character Certificate) from your local police station.

Go to your nearest Passport Office with your original CNIC and required documents. Fill out the passport application form, provide fingerprints and photo, and pay the passport fee. You can choose normal or urgent processing depending on how quickly you need it.
